One page, 13 3/8 x 17 in., folds, light toning, Madison's signature a bit weak, with embossed seal at bottom.

Appointing Richmond Johnson a surgeon in the United States Navy. Johnson had served in the War of 1812 as a Surgeon's Mate in the Navy before his to promotion to full Surgeon in 1815. He resigned from the service in February 1817.
